Overview:

This document outlines the terms and conditions governing the purchase of nutrition sessions and packages offered by Dana Monsees Nutrition.

Billing:

If you elect to pay by session, you will be charged with the card on file at the beginning of the session. If there is an issue with the card on file, this must be resolved before the session continues. 

If you elect to pay in installments, packages are charged on a monthly basis; with the monthly installment payment calculated based on the chosen package. See package for breakdown with the payment calendar. 

Package Options:

The packages available for purchase are as follows: 6-session package, and 12-session package.

Package Expiration:

  • 6-Session Package: The 6-session package is valid for a period of EIGHT [8] months from the date of purchase.
  • 12-Session Package: The 12-session package is valid for a period of FIFTEEN [15] months from the date of purchase.

Unused Sessions:

Any unused sessions at the end of the validity period will expire and cannot be carried forward. No refunds will be issued for unused sessions.

Scheduling:

Sessions must be scheduled in advance and are subject to availability.

Cancellations and rescheduling must be done [72] hours in advance of the session time and date; otherwise, the session will be forfeited and the client will be responsible for the cost of the session. For example, a 12pm session on Monday must be canceled or rescheduled by Friday at 12pm.

Session “No-Show” Grace Period

If the client does not appear for the session within 15 minutes of the scheduled start time, the session will be forfeited and the client will be responsible for the cost of the session. 

Termination:

The client may cancel the remaining sessions in a package at any time, but no refunds will be provided for unused sessions.

Session Extensions:

In exceptional circumstances, such as health-related issues or unforeseen emergencies, the client may request an extension of the package or rescheduling of the session. Such requests will be considered on a case-by-case basis.

Package Upgrades:

Clients may upgrade to a higher session package at any time by paying the difference in fees.

Changes to Terms:

Dana Monsees Nutrition reserves the right to modify these terms and conditions at any time. Clients will be notified of any changes.

Legal Disclaimer:

These terms and conditions are a legally binding agreement between Dana Monsees Nutrition and the client. By purchasing a nutrition session package or booking a session with Dana Monsees Nutrition, the client confirms their agreement to abide by these terms.

CANCELLATION & RESCHEDULING POLICIES

Dana Monsees Nutrition REQUIRES 72 HOURS NOTICE FOR CANCELLING OR RESCHEDULING AN APPOINTMENT. IF YOUR APPOINTMENT IS ON A MONDAY, YOU MUST CANCEL 72 HOURS IN ADVANCE (ON FRIDAY). If you wish to cancel or reschedule, you must do so before 72 hours of your appointment time.

IF PRIOR NOTICE IS NOT GIVEN, YOU WILL BE CHARGED THE FEE ASSOCIATED WITH THE SCHEDULED APPOINTMENT. SIGNING THIS AGREEMENT CONFIRMS YOUR CONSENT TO THESE TERMS.

NO-SHOW & GRACE PERIOD POLICY:

Governing Law:

This agreement is governed by the laws of Maryland/United States of America.

TERMS AND CONDITIONS – NUTRITION SESSIONS

I have sought the clinical and health care services of Dana Monsees Nutrition – for my personal healthcare or for my child or children who are minors. I understand that this health practice uses some approaches and methods that are known as complementary, alternative, holistic or functional in nature. This may not be covered by my insurance plan. The terms complementary, holistic, alternative or functional refer to therapies that may include, but are not limited to, dietary and nutritional supplement advice, certain dietary/exercise protocols to follow, and certain functional tests (stool testing, hair testing mineral analysis), blood work panels, etc. that are used for informational purposes. 

Foods, vitamins, minerals, enzymes, herbs, and other nutritional approaches may also be chosen as adjunctive to medical therapies you are currently engaging in with your doctor or other health practitioners. It is your responsibility to ensure you inform your medical doctor of all supplements/diets you will be partaking in so that he/she can make sure there are no contraindications to your medicines.

Our nutrition programs are online-based. We are not affiliated with a local hospital. As a result, WE STRONGLY RECOMMEND THAT IN ADDITION TO OUR SERVICE YOU MAINTAIN A RELATIONSHIP WITH ONE OR MORE DOCTORS QUALIFIED TO CARE FOR YOUR INDIVIDUAL HEALTH CONDITIONS. We often refer clients to these and other healthcare professionals when it is deemed necessary. We are happy to communicate and cooperate with your doctor(s), therapists, or other practitioners regarding your medical condition(s), options or any other health related issues.

As with many health related services, there are certain potential complications which may arise during the receipt of these services. Those complications range from discomfort through serious health concerns requiring emergency medical care. The probability of these complications are rare but you are being made aware of the full range of possibilities that may occur and assume the risk of proceeding with care by signing this agreement.

Dana Monsees Nutrition makes no representations, claims, or guarantees regarding the efficacy of our recommendations. The protocols we recommend are based upon a combination of our clinical experience and knowledge of scientific and medical literature. With this information individualized protocols may be offered and applied as either adjunctive or primary protocols for certain conditions. The undersigned is also expressly notified that the practitioner providing training and nutritional services is certified as a Board-Certified Nutrition Specialist (CNS) by the American Nutrition Association / Board of Certified Nutrition Specialists and Licensed Dietitian Nutritionist (LDN) by the State of Maryland.

By signing this informed consent you agree to hold harmless Dana Monsees Nutrition and its practitioners from all professional and personal liability, negligence, or other legal liability. You agree to be responsible for all legal costs and fees that may result from action(s) on your part or on the part of your representative(s) against us. If a legal case is brought against us, you agree that we shall be judged by the standard principles of complementary/holistic/alternative/functional medicine and not the standards and principles of consensus of conventional/allopathic medicine. You have the right to have this consent reviewed by your lawyer before accepting any services from our office and we suggest that you exercise this right.

Most insurance plans cover services that they consider medically necessary and/or reasonable and customary. Many of our services such as nutritional consults, dietary protocols and testing (blood/urine/saliva/stool) are often not considered by insurance companies to be necessary or a “covered service” and, therefore, reimbursable, based upon their own criteria. By signing this form you accept full financial responsibility for all non-covered services; including consultations, , blood/saliva/urine/stool and other laboratory tests and procedures. Your signature verifies and affirms that you have not been told to discontinue treatments with any other medical specialists or other health care providers.

Your signature is being given prior to rendering any services, advice, and/or recommendations whatsoever from Dana Monsees Nutrition.

Dana Monsees Nutrition does not allow their sessions with any client to be recorded on any kind of device. If a client wants to record a session Dana Monsees Nutrition has to give its consent.
